Each student-athlete will need an updated physical signed and dated by a medical physician on per MHSAA guidelines. The physical along with other information will need to be uploaded electronically starting this school year. Below is our welcome message to our new electronic database.
We are very excited to announce that the Edsel Ford Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. Final Forms alo pre-populates information wherever possible, for each year of your students, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
Click on the Parent Playbook (attached) to get started in FinalForms. If you require any support, scroll to the page bottom and click “get help”.
We are asking that ALL parents and athletes use FinalForms. Please do not wait to get signed up and complete all information as this could impact the student-athletes ability to participate on August 11.

Fall Athletics begin today on Monday, August 11th. Each student-athlete trying out for a sport and their parents/guardians will need to complete all electronic paperwork and attach a physical prior to beginning sports.
