INSTRUCTIONS:
- Be sure you print the age appropriate form
- Both sides of the form must be printed for it to be valid
- Read the instructions on the back of the form BEFORE you fill it out
- Sections I and II must be completed before returning the Work Permit to the school
- When school is in session, bring the completed form to the Student Office (Main Office during the summer) for a permit # and signature
